高血圧性脳内血腫のCT誘導定位脳手術46症例の追跡調査

A New Treatment for Hypertensive Intracerebral Hematoma: A follow-up study on 46 patients with hematoma treated by CT guided stereotactic method

I．はじめに

　近年，侵襲の極めて少ないCT誘導による定位脳手術を用いた脳内血腫除去法が開発され臨床応用されるようになった．この手術は，局所麻酔下に正確かつ安全に血腫を除去でき，開頭による血腫除去とは別の見地から高血圧性脳内出血に対する治療手段としての本手術法の評価が必要となってきた．そこで今回著者らは，CT誘導定位脳手術により脳内血腫除去を行った46症例について，血腫除去率および手術成績について詳細に検討したので報告する．

Durinig the last 2 years, 46 cases of hypertensive intracerebral hemorrhage in the basal ganglia were treated by CT guided stereotactic aspiration and their outcome was evaluated in terms of the rate of hematoma removal, the change of conscious-ness level and the recovery of motor and sensory functions. They are aged from 45 to 79 years old, the average 56, and aspirated 1 to 24 days after the onset, two third of them being within 1 week. The whole procedure was done in the CT room under direct CT guidance and by one trial.
