急性期重症脳障害患者の予後判定—聴性脳幹反応（ABR）と瞬目反射（BR）による評価

An Assessment of Outcome Prediction in Patients with Severe Brain Damage with Auditory Brainstem Response and Blink Reflex

I．はじめに

　近年，各種誘発電位の分析を指標とした重症脳障害患者における予後評価が盛んに試みられている．Green-bergら^1-3）は重症頭部外傷の予後判定にmultimodality　evoked potentials（MEPs）が有用であると報告している．しかしながら重症脳障害患者の急性期にこれら各種誘発反応を同時に，しかも経時的に記録することは種々の制約があり必ずしも容易でなく，Greenbergらが論述しているほど有用でない場合が少なくない．

　そこで著者らは，各種誘発反応のうちから，聴性脳幹反応（auditory braintem response,ABR）と三叉神経第1枝の電気刺激により誘発される瞬目反射（blink re-flex,BR）とを予後評価の指標に選び，それぞれ脳幹の異なる経路を介して形成されるこの2つの反応を分析することにより重症脳障害患者の急性期における予後評価がどの程度まで可能か，その有用性と限界について検討したので報告する．

Results of auditory brianstem response (ABR) and electrically elicited blink reflex (BR) recorded from 43 patients with severe brain damage within three days after the onset of illness were analyzed to assess the prognostic value of ABR and BR with respect to patient outcome evaluated by the criteria proposed by Jennett and Bond.

It was possible to recognize, in recordings obtained from patients with severe brain damage, three basic patterns of BR as well as five patterns of ABR within three days after the onset.
