呼吸麻痺を伴う上位頸髄損傷—リハビリテーションの段階に至った1生存例

A Case of Quadriplegia with Respiratory Paralysis who survived as the Candidate for Rehabilitation

I．はじめに

　第4頸髄以上の上位頸髄に対する重度の外傷性損傷は呼吸麻痺をきたすことが多いために致命的な外傷といわれている^5）．現場での救急医療体制のいまだ不充分なわが国では，そのほとんどが死の転帰をとっていると考えられ，受傷直後から呼吸麻痺を伴う上位頸髄損傷患者の蘇生に成功し，社会復帰を目指す段階にまで回復した症例の報告は，われわれの渉猟し得た範囲では見られない．最近われわれは，交通外傷による環椎前方脱臼を伴う軸椎歯状突起骨折を生じ，心肺機能停止状態で救急搬入されながらも蘇生し，吸呼麻痺を伴う四肢麻痺の状態から病状の目覚しい改善を示した症例を経験した．同部の損傷は神経外傷学的な立場から見ても，いくつかの特徴^6,10）があり，本症例もその経過中興味ある所見を呈したので併せて報告する．

A case of a 20-year-old woman who survived res-piratory quadriplegic state and then became to be a candidate for rehabilitation is reported. The patient was injured in an automobile accident and immedi-ately developed cardiopulmonary arrest, quadriplegia and maximum mydriasis bilaterally. She was trans-ferred to our hospital six minutes after the accident. Immediate and appropriate resuscitation could bring restoration of only cardiac contractions without spon-taneous breathing and minimal movement of extre-mities.
