対称性かつほぼ同時に発生した高血圧性脳血出の2症例

Simultaneous, Bilateral Hypertensive Intracerebral Hematomas

I．はじめに

　高血圧性脳内血腫の発生部位，頻度，病態，予後，手術適応，血腫の再発時期などについては，CT出現以後，詳細な発表が多く見られるが，多発性のものや再発例の報告は少なく，さらに両側半球にわたってほぼ対称性に血腫をもつものは極めて珍しく，同時発生例に至っては数例の報告を見るにすぎない．

　著者らは最近，ほぼ同時発生と推定しうる両側視床出血，および比較的短時間内に両側大脳基底核部に出血した例の計2症例を経験したので，若干の文献的考察を加え報告したい．

The reported incidences of bilateral intracerebralhemorrhages clue to systemic arterial hypertensionare exceptionally rare in Japan. Unilateral hemor-rhages, on the other hand, are less uncommon. Re-cently, we have examined two patients with bilateralintacerebral hemorrhages due to hypertension. Thefirst case involved bilateral thalamic hemorrhages;and in the other, a contralateral hemorrhage deve-loped postoperatively, subsequent to the evacuationof a primary hematoma. The characteristic neuro-logical manifestation of bilateral intracerebral hemor-rhages include quadriparesis, bilateral Babinski's signs,stupor, and coma.
