巨大血栓化脳動静脈奇形の1症例

Totally Thrombosed Large Arteriovenous Malformation: A case report

I．はじめに

　脳動静脈奇形（AVM）の一部が，血栓によって閉塞することのあることは，Norlén, Rubinstein, Noran, Raskinその他の手術所見，病理所見により古くから知られ，このような症例では脳血管撮影上，AVMが造影されないで，avascular massとしてとらえられる場合のあることも知られている^{2,5-7,11,17）}．しかし，このような脳AVMがmass effectを呈することは，AVMからのmassive hemorrhageを来たした時以外には極めて稀である．著者らはそのほとんどが血栓化，器質化した巨大なAVMで，massive hemorrhageを伴わず，またdural AVMで見られるような髄液還流不全など^15）を示さないで，AVMそのものが頭蓋内占拠性病変として働き，脳室の圧排変形とmidline structureの偏位を来たした症例を経験したので報告する．

A totally thrombosed large arteriovenous malformation(AVM) which operated as an intracranial space occupyinglesion was presented.

The patient was a 24-year-old woman who had hadfrequent attacks of loss of consciousness since the age of 13years and had been on anticonvulsant medication at thepsychiatric department of our hospital. She was referredto our clinic with bitemporal girdle pain, nausea and vomit-ing.

Neurological examination revealed right homonymoushemianopsia, sensory aphasia and choked discs in bothoccular fundi.
