After clipping for the anterior communicating artery aneurysm a newly formed aneurysm was noticed in the postoperative angiogram.

A 32-year-old man was admitted to the hospital with the symptoms of subarachnoid hemorrhage. Four vessels angiography was performed and it revealed an anterior communicating aneurysm and hypoplasia at the A1 portion of the rt. anterior cerebral artery. The neck of the aneurysm was clipped under a surgical microscope through the fronto-basal-lateral approach.
