It had been reported by Drake that in vertebral aneurysms at the origin of the posterior inferior cerebellar artery (PICA), there are a few cases, in which because of size and position it is deemed advisable to occlude one of vertebral arteries. Further more, he stated, the results of vertebral artery occlusion was encouraging and the technique deserved further consideration.

In our service one of vertebral arteries was occluded proximal to the origin of the PICA for the treatment of the vertebral aneurysm at the origin of the PICA in two cases successfully. Postoperative courses of both cases were uneventful.
