The Usefulness of the Twist Drill Technique for Conray Ventriculography

Ⅰ．はじめに

　1940年代すでにConeらによりtwist drill techniqueが開発，施行されていたが，危険性が多いことの理由で公表されなかった．1966年Rand^5）らは熟達したneurosurgeonであることを条件に，その迅速性，簡便性を強調し，脳室造影，脳室ドレナージ，頭蓋内血腫の診断および治療などに有用性を持つことを発表した．その後本法が諸施設で行われて来ているが，頭蓋内操作がblindであるため，安全に対する危惧感から，緊急時以外施行されぬ場合が多い．

　我々は1968年以来，約200例に本法を施行し，特にconray ventriculographyに応用しているが^3），考えられる危険性に遭遇していない．我々の方法を紹介し，その経験から2，3の考察を述べてみたい．

The twist drill technique is simple, quick and safe to perform. We have used this technique in 200 cases of conray ventriculography, and found it a very useful procedure. The procedure is as follows :

The patient is placed in the supine position with the head on a skull table. A small area of the scalp, about 3 cm in diameter, is shaved on the right frontal region around the site of puncture, which is 7 cm superior to the nasion and 2.5 cm lateral to the midline. After infiltrating the area with 0.5% Xylocaine containing epinephrine, a small stab wound (less than 0.5cm in length) is made through the scalp and periosteum.
