Bilateral inferior horizontal hemianopsia caused by an ectopic pinealoma was experienced. The case was a 17-year-old girl, who presented inferior horizontal hemianopsia, decreased visual acuity, optic atrophy bilaterally and panhypopituitarism. She had no other signs. The pneumoencephalography showed normal posterior third ventricle, but the ventriculography showed a filling defect of the anterior third ventricle and the enlarged lateral ventricles. The operation revealed that the tumor almost obliterated the foramen of Monro. Histologically it was a two-cell pattern pinealoma.
