Microsurgeryを応用したSpondylotic Vertebral Artery Insufficiencyの手術法—手術手技についての最近の進歩と25例の術後遠隔成績について

Microsurgical Treatment of Spondylotic Vertebral Artery Insufficiency

　頸部脊椎症による椎骨動脈不全（Spondylotic vertebral artery insufficiency）の手術法には，いろいろの方法がある^1-10）．1967年，著者は独自に屍体を用いて，これらの手術法について検討した結果，すでに発表した《Uncectomy, unroofing of transverse foramen, and periarterial denervation　（stripping）》が，本症の手術方法としてもっとも合理的であることを知った．この方法を1969年脳と神経^9）に，1970年Journal of Neurosurgeryに発表したが^10），1967年当時の初期の頃には，MicrosurgeryもHall Air Drillも用いていなかった．しかし，最近ではMicrosurgeryを応用することによって，深部のこまかい操作が，安全にしかも確実に行なえるようになり，この手術法も若干進歩したように思う．ここに，その後の改良と進歩を中心に，現在著者が行なっている手術方法を述べてみたい．なお，本法はYear Book1971^11）とAir Instrument Surgery VoL．1^12）に紹介されているが，Air Igstrument Surgeryでは，図が少し正確でないところがあるので，この際，訂正する意味でHall Air Drillを使った本手術法の図を作成した．

In earlier report, the author described an operation for vertebral artery insufficiency due to cervical spondylosis that evolved from cadaver studies in cases of arterial displacement due to severe osteophtes on the uncinate part of the vertebrae. The method consisting of three steps, uncectomy, unroofing of transverse foramen, and periarterial stripping had been used since 1967. Subsequent 7 years' experiences in this surgery, now justifies a further report detailing several technical advances with use of microscope and new instrumentation.
