筋萎縮を伴う重症筋無力症（いわゆるOsserman-Ⅴ型）の臨床病理

まえがき

　重症筋無力症は障害部位，臨床経過，重症度などの点で必らずしも同一の病像を示さない。また，本症では臨床的に筋萎縮を認めない場合でも生検あるいは剖検筋の検索により顕微鏡下には萎縮変性（神経原性および筋原性）が高率に認められる^{1，2，4，5，10，11，15，16）}。

　しこうして臨床的に明らかな筋萎縮を合併する本症をOssermanは別個に取り扱い分類（Ⅴ-group）しているが，このような非定型的症例では他の神経筋疾患との相関（あるいは合併）がとくに重要視されている。

　Among 169 cases of myasthenia gravis examined during these 18 years (1951-1969) at the 3 rd Dept. of Int. Medicine, University of Tokyo, we investigated 37 cases accompanied with muscular atrophy clinicopathologically.

　Ten cases of them complicate tongue atrophy or both tongue and other skeletal muscular atrophy; the feature is that those are seen mostly in females aged twenties or thirties, complication of thymoma is rare, and that atrophy begins from the peripheral part of tongue.
