高血圧性脳出血の外科的治療に関する臨床神経学的研究—特に血腫部位診断ならびに手術適応の観点より

Ⅰ．緒言

　高血圧性脳出血の手術は1903年Cushingにより最初に報告され，それ以後諸家により少数例が報告されてきたが，1950年以後報告者の数も増し，また症例数も増加してきた。それらのうち特に欧米ではDavidoff^6），Fisherg^9），Luessenhop^23），McKissok^31,32）らが詳細に報告しており，わが国においては光野^25〜30），金谷^8〜22）の報告がみられる。これら報告のうち神経学的所見と本症の出血部位とを対比して詳細に観察し，しかも血腫別除後それらの推移に関し研究した報告は殆どない。

　そこで著者はそれらを明らかにするとともに，術前神経学的所見より血腫部位を判定して手術適応決定上の指標となりうるかどうかについて臨床神経学の立場より，その意義について検索し興味ある知見をえたので報告する。

Neurological examinations for preoperative determination of hematoma location were performed on the admitted and operated patients in an acute stage of hypertensive intracerebral hemorrhage and neurological findings after the evaluation of hematoma were studied.

　(1) Disturbance of consciousness was severe and frequent in combined type patients and mild in lateral type patients.
