Posttraumatic persistent vegetative state

はじめに

　頭部外傷による意識障害は決して珍しいものではない。たとえば脳振盪などは受傷時にしばしばみられる短時間の，可逆的の意識喪失である。したがってこの場合の意識障害は，明らかに外力によって脳の一部になんらかの変化が惹き起こされたことが原因であることは，疑う余地のないところである。しかしその際にみられる脳の形態・機能両面の変化の詳細や，その部位に関してはなお未解決である。

　一方，受傷当時から脳の一定部位，とくに脳幹に重篤な一次的の損傷が加わったような場合には，脳振盪などにくらべてはるかに長時間にわたる，時には非可逆的な意識障害がみられることがある。また頭蓋内出血や脳浮腫などにより，主として頭蓋内圧亢進→脳循環障害→脳無酸素症を経て，二次的に意識障害が起ってくる場合も珍しくない。

Eight cases with persistent vegetative state due to head injuries were presented. Based on theclinical and operative findings of those cases and the pathological findings heretofore reported in the literatures, the localization of cerebral damage, coma mechanisms, treatment and prognosis were discussed.

As for the localization of cerebral damage, no special structures might be involved in the mechanism of posttraumatic vegetative state.
