Studies on H-reflex elicited from human hand muscles

Ⅰ．Tetanizationによる上肢筋からのH波の誘発

　1918年にHoffmann^22）よって発見され，1950年Magladery and McDougal^31）によって詳細に研究されたH波は，ふつうposterior tibial nerveに電気刺激を加え，M．soleusおよびM．gastrocnemiusから誘発されるものである。このH波を用いて脊髄運動神経細胞の興奮状態を調べる研究がこれまでいろいろと行なわれてきたが，単にこのような方法だけで調べられるのは，H波の出やすい下肢の筋肉を支配する運動神経細胞のみであって，上肢の筋肉からは，ふつうの状態では，H波を誘発することが困難であるため，そのままでは臨床的に上肢筋を支配する運動神経細胞の興奮状態を調べることはできない。

1. Post-tetanic H-reflex from thenar muscle was recorded in 55% of normal subjects and 86% of patients with upper motor neuron disturbances.

2. The maximal amplitudes of the H-reflexes were obtained 15 to 30 sec after tetanization, and the H-reflexes showed gradual attenuation and disappeared within a few minutes. The post-tetanic H-reflexes elicited in patients with upper motor neuron disturbances had tendency to last longer than those from normal subjects.
