視覚前兆の神経生理学

Neurophysiology of visual aura in migraine.

　片頭痛における視覚性前兆は，神経説においてその特徴的な発現様式から動物実験で明らかにされた拡延性抑制（cortical spreading depression：CSD）に基づき考えられてきた。近年の神経機能画像の進歩により，視覚性前兆とCSDの関連がより明らかにされつつある。一方，発作間歇期の神経生理学的研究では，視覚路における潜在性の機能異常が存在することが報告されている。このうち視覚誘発電位の研究では，その特徴的な所見から視覚野の易興奮性，慣れの障害，視覚情報の処理過程における障害の三つの病態が存在することが考えられる。片頭痛ではこのような病態がCSDを発現させる要因の一つとなるものと推測される。

This paper reviews clinical literature relating to neurophysiology of visual aura in migraine. Recent researches have focused on the relationship between the interictal status of visual neural networks, the physiological events of cortical spreading depression (CSD), and the phenomenology of the visual aura. The visual aura of the migraine attack has been explained by the CSD by Leao, a neuroelectric event beginning in the occipital cortex and propagating into contiguous brain region.
