痙性斜頚の手術治療

Surgical management of spasmodic torticollis.

痙性斜頚の多くは頚部筋群のジストニアであると考えられており，異常収縮筋の大半は副神経支配筋とともに板状筋，半棘筋などの頚髄神経後枝の支配筋である。外科的治療として，これらの筋肉への運動神経を末梢で選択的に遮断する選択的末梢神経遮断術の有効性が確立されている。本手術はボツリヌス毒素局注と同様な神経遮断を外科的に行うもので，対症療法ではあるものの効果は確実で副作用も少ない。本手術はBertrand原法から，ここで紹介するTaira変法への変遷によってさらに侵襲が少なくなり，80例の自験例ではボツリヌス毒素局注以上の効果が得られている。この手術方法や術前後の管理についての詳細を記述し，選択的末梢神経遮断術が痙性斜頚の治療の選択枝の一つとして考慮しておく必要があることを強調したい。

Majority of spasmodic torticollis is considered to be an idiopathic cervical dystonia, mainly involving the mus-cles innervated from the accessory nerve and the posterior branches of the cervical spinal nerves. Selective sec-tioning of the peripheral motor nerves to the involved muscles is an established surgical treatment known as se-lective peripheral denervation. The Taira's refined procedure of the original Bertrand method gives better results in terms of invasiveness and side effects.
