Silent brain infarction (SBI) became to be considered as an important predictor for stroke since several reports that confirmed significant incidence of SBI on CT/MRI in first-ever stroke. Recent studies using MRI showed SBI as 88% in hypertensive hemorrhage. Silent white matter hyperintensity was also found in asymptomatic hypertensive elderly. The SBI was mostly small lacunar infarction located in basal ganglia and in deep white matter (Fig. 1). It should be differentiated with enlargement of perivascular space (Fig. 2).
