前頭葉内側面損傷と手の把握行動

Manual grasping behavior in patients with medial frontal lobe damage.

前頭葉，とくにその内側面の損傷後に病巣と対側の手に把握反射，本能性把握反応，imitationbehavior，道具の強迫的使用などさまざまな異常行動が生じることが報告されている。しかしそれらの責任病巣および発現機序についてはほとんど分かっていない。筆者らは梗塞あるいは出血にて一側大脳半球内側面が損傷された34例において，発症6週間以内に病的把握現象とそれに関連した異常行動を観察し，MRIにて病巣部位との対応を検討した。その結果，把握反射は主として対側の補足運動野の障害により，本能性把握反応の中のmagnet reaction, visual gropingは対側の前部帯状回の中―後部（脳梁体部周囲）の障害により，imitation behaviorは対側の前部帯状回の前部（脳梁膝部周囲）の障害により，道具の強迫的使用は対側の前部帯状回の前，中―後部の広範な障害により生じると思われた。

I investigated pathological grasping phenomena and related prehensile behavior in 34 patients with recent damage to a unilateral medial hemispheric wall from stroke. Phenomena were categorized as either the grasp reflex or the instinctive grasp reaction. The instinctive grasp reaction was subdivided into closing reaction, final grip, trap reaction, magnet reaction and visual groping. To determine the exact location and extent of the lesions, I examined magnetic resonance images.
