The importance of cytotoxic T cells (CTL) in the autoimmune inflammatory myopathies has been emphasized recently. Previous studies showed that CTLs invading into non-necrotic muscle fibers are the hallmark of polymyositis. The molecular mechanism of CTL-mediated muscle fiber injury has not been understood to date. Here we demonstrate for the first time that one of the cytolytic enzymes in the CTL granules, termed granzyme A, not only degrades a muscle cytoskeletal protein, dystrophin, but also degrades muscle structual proteins, such as myosin heavy chain, myosin light chains, and nebulin in vitro. So we postulate that granzyme A invades into a muscle fiber through the pore which perforin has made, and hydrolysis several components of muscle fiber and which finally leads to the muscle fiber necrosis. Our histochemical study further confirms this, showing predominance of CDS+ perforin granzyme A + mononuclear cells in the perimysium and endomysium of polymyositis/inclusion-body myositis.
