　57歳，男性。左陰嚢内腫瘤を主訴に当科受診。触診，陰嚢エコーにて精巣腫瘍が疑われ，左高位精巣摘除術を施行した。病理組織では，精索から精巣上体にかけて多発する中分化腺癌であり，転移性の腫瘍が疑われた。CTにて膵尾部に腫瘍を認め，脾臓への浸潤が疑われた。外科にて膵尾部と脾臓を合併切除し，その病理組織はいずれも中分化腺癌であり，精索のものと同様であった。膵癌の精索転移は稀であり，本症例は本邦11例目にあたる。

A 57-year-old man consulted our hospital complaining of right scrotal swelling. Physical findings and ultra-sonography revealed testicular and spermatic cord tumors. Right inguinal orchiectomy was performed and the pathological examination revealed metastatic adeno-carcinoma. CT scan demonstrated original tumor at the pancreas tail with splenic invasion. Pancreas tail and spleen were removed and the pathological findings revealed moderately differentiated adenocarcinoma, identical to that of spermatic cord tumor. This is the 11th case of metastatic tumor of the spermatic cord from pancreas in Japan.
