顆粒球コロニー刺激因子（G-CSF）産生膀胱腫瘍の1例

Bladder Carcinoma Producing Granulocyte Colony Stimulating Factor:A Case Report

　78歳，女性。肉眼的血尿を主訴に受診した。膀胱右側壁に4cm大，頂部に2cm大の浸潤性腫瘍をそれぞれ認めた。入院時の末梢白血球数は45,000／mm³であった。脳梗塞の既往を有し，ねたきりの状態で痴呆症もあるため姑息的にTUR-Btのみを行った。TUR-Btの翌日の白血球数は18,900／mm³と一時的に下降したが，以後は徐々に上昇した。全身状態も悪化して，TUR-Btの2か月後に癌性悪液質のため死亡した。血清G-CSF濃度は138pg／mlと高値を示し，G-CSF特異抗体を用いた免疫染色で腫瘍に陽性所見を得たため，本邦17例目の願粒球コロニー刺激因子（G-CSF）産生膀胱腫瘍と診断した。

A 78-year-old woman with invasive bladder tumor was admitted to our hospital complaining of gross hematuria. In her peripheral blood leukocytosis of 45,000/mm³ and elevation of serum G-CSF concentration (138 pg/ml) was shown. TUR of the tumor revealed transitional cell carci-noma, G-3. The tumor tissue had positive immunohis-tochemical staining for G-CSF. This is the 17th reported case of G-CSF producing bladder tumor in Japan.
