68歳男性，全身倦怠感と不明熱で受診する。初診時検査で白血球増加，CRP陽性であった。造影CT・MRIで，右腎上極部に2.5×3.1の造影効果のある腫瘍を認めた。手術は経腹的に右腎全摘除術を施行した。病理診断はclear cell type，INFγ，pT4であった。術後も白血球増加，CRP高値は続き，2週間後には左臀部痛出現，左腸骨に転移を認めた。治療効果なく，術後36日目に永眠された。病理特殊染色検査でG-CSF抗体陽性であり，腎摘術後急速に進行したG-CSF産生腎腫瘍と診断した。

The patient was a 68-year-old man with the chief complaint of high fever and general fatigue. Computed tomography revealed right renal tumor. Laboratory date showed marked leukocytosis and increased serum CRP. Right nephrectomy was performed and pathological report showed renal cell carcinoma. Immunohistochemical staining using anti-G-CSF antibody showed positive in the cancer cells. The postoperative course was very rapidly progressed. The patient died 36 days after the operation.
