94歳男性に認めた精母細胞性セミノーマ

Spermatocytic Seminoma with 94 Year-old Male

　94歳，男性。1997年2月頃より右陰嚢の無痛性腫大に気付き，4月15日に当科外来を受診した。右精巣は小鶏卵大に腫大し，超音波検査では嚢胞状腫瘤に—部充実性の部位を認めた。4月21日，右高位精巣摘除術を施行した。摘出精巣は60×33×20mm大で，70gであった。病理組織学的には精母細胞性セミノーマであった。超音波検査やCT検査にて明らかなリンパ節や他臓器への転移はなく，年齢も考慮して経過観察中である。本疾患は比較的稀であり，セミノーマの好発年齢は30歳代であるが，精母細胞性セミノーマは40歳以降に多いとされている。筆者らの調べ得た限りでは，本症例は本邦最高年齢と思われた。

A 94-year-old male presented complaining of persistent painless swelling of his right scrotum for two months in April, 1997. The testis was swollen to the size of a chicken egg, and an ultrasound examination disclosed solid mass. High inguinal orchiectomy was performed and the testis was 60×33×20mm in size and weighted 70g. Histopatho-logic examination showed spermatocytic seminoma. Me-tastasis to the lymph nodes and other organs was not found by CT and ultrasound examinations. Because of his age, the patient was put under follow-up observation.
