対側精管精嚢欠損症を伴った精嚢嚢状拡張症

A CASE OF CYSTIC DILATATION OF THE SEMINAL VESICLES WITH AGENESIS OF CONTRALATERAL SEMINAL VESICLE AND VAS DEFERENS

　症例は39歳，男性。不妊を主訴に受診した。精液検査にて無精子症。右精管欠損および左精嚢の著明な拡張を認めた。1996年1月，射精管拡張術を施行したが精子は得られなかった。精嚢嚢状拡張症は，両側性で嚢胞腎との合併が多いとされているが，片側性で対側精管精嚢欠損があるものは本例が本邦2例目であった。

A 39-year-old man presented with infertility. Semen analysis showed azoospermia. The patient had cystic dilatation of the left seminal vesicles with agenesis of contralateral seminal vesicle and was deferens. He was treated with retrograde dilatation of the ejaculatory duct, but sperm was not detectable. Cystic dilatation of the seminal vesicle has two characteristics, namely the involvement of bilateral seminal vesicles and often as-sociated with polycystic kidney disease. To our knowl-edge, this is a second case of unilateral seminal cystic dilatation with agenesis of contralateral seminal vesicle and was deferens in Japan.
