In situでの体外衝撃波砕石術により治療に成功した後部尿道結石

A case of posterior urethral stone successfully treated by in situ ESWL

　症例は49歳，男性で，右側腹部痛を訴え受診し，KUBにより右尿管結石と診断された。翌日，排尿困難および会陰部不快感を訴えた。KUB，超音波検査およびCT検査により結石が後部尿道に下降したことが判明した。後部尿道結石は，鎮痛剤なしで，EDAP LT-02^®を用いたin situでの体外衝撃波砕石術（in situ ESWL）で治療した。焦点合わせは，患者を仰臥位にし，X線透視で行った。骨盤骨による衝撃波の遮断を最小限にするため，衝撃波は片側臀部外側から発射した。ESWLによる治療後，破砕された結石は疼痛なく自然に排出された。本症例により，in situ ESWLは，後部尿道結石の治療の選択肢の一つとなる可能性があることが示唆された。

A 49-year-old man with a right ureteral stone complained of difficulty in voiding and perineal discomfort. Pelvic X-ray, sonography and CT scan revealed that the stone had moved to the posterior urethra. Posterior urethral stone was treated by in situ extracorporeal shock wave lithotripsy (ESWL) using an EDAP LT-02 lithotriptor without analgesia. Focusing shock wave was performed with the patient in the supine position using fluoroscopy. Shock waves were projected through the unilateral hip side to minimize the obstruction of the shock waves by the pelvic bones.
