前立腺全摘10年後に吻合部再発をきたした前立腺癌

A Case Report: Anastomotic Recurrence 10 Years after Radical Prostatectomy due to Prostatic Cancer

　69歳，男性。1988年10月前立腺全摘施行。被膜浸潤，尿道側断端陽性のため両側精巣摘除術を追加した。1997年1月正常値下限であったPSAが3.1ng／mlと上昇し，1998年10月直腸診で吻合部に腫瘤を触知した。生検を施行し，低分化型腺癌であったため抗アンドロゲン剤投与および放射線治療を行った。腫瘤は縮小し，再生検でも癌細胞の空胞変性を示した。2000年8月現在，PSAは正常で画像上も再発を認めていない。

A 59-year-old man underwent radical prostatectomy for prostate cancer in 1988. Adjuvant bilateral orchiectomy was performed because capsular and distal urethral edge invasions. Minimum level of PSA maintained and then serum PSA elevated to 3.1ng/ml in January, 1997. Digital rectal examination revealed tumor recurrence at the vesico-urethral anastomosis in October, 1998. Both transurethral resection and transrectal needle biopsy of the mass showed poorly differentiated adenocarcinoma exhibiting extensive lymphangial and venous invasion.
