細径のシース，ループ，内視鏡を用いた経尿道的切除術

The Experiences of Transurethral Resection Using Slender Resectoscope Systems

　従来筆者らが用いていた切除鏡システムのシース（外筒）は24Fr以上であり，挿入や操作に際して尿道の抵抗が強いことも多いため，1998年6月より18Frの細径シース，17Fr相当のループおよび9Frの内視鏡を用いて経尿道的前立腺切除術（TUR-P）および膀胱腫瘍切除術（TUR-Bt）を施行した。TUR-Pは21例に施行し，切除片は小さく，灌流液の流入も少なかったが，尿道からの擦過出血や術後狭窄を軽減することができた。TUR-Btは7例に行ったが，筋層浸潤している膀胱腫瘍のデリケートな切除も可能で，今後有用なシステムと思われた。

The size of resectoscope sheath usually used for transure-thral resection of prostate (TUR-P) or bladder tumor (TUR-Bt) is 24 French (Fr) or larger. However, we considered this size too large for most Japanese urethra, and we started to use smaller sized-resectoscope systems in June 1998. We performed 21 TUR-P. The tips were small, the irrigant volume was limited, and the telescopic field was also limited. However urethral resistance at inser-tion and during manipulation was decreased. In 3 TUR-Bt cases, with the small tips, we could resect the tumor more delicately especially in the muscle layer.
