真性包茎の包皮先端部に生じたBowen癌

A case of Bowen's carcinoma at the tip of foreskin with true phimosis

　症例は，95歳，男性。真性包茎。陰茎包皮先端部全周性に一部にびらんを伴う紅色結節あり。生検施行しBowen病と診断。環状切除術の要領で腫瘍切除術を施行。病理組織標本では真皮内への浸潤像がみられ，Bowen癌と診断した。HPV免疫染色は陰性であった。全身検索にて転移像はみられず，経過観察中である。過去10年間の本邦報告例の集計では，外陰部に生じたBowen病は浸潤癌になりやすく，注意が必要と考えられた。

　We report herein a 95-year-old male with Bowen's carcinoma of penis with true phimosis. He complained red papule at the tip of foreskin,and a biopsy showed that the patient had a Bowen's disease. Circumcision was performed for tumor resection. Histopathological findings were Bowen's carcinoma,because of invasion to dermis. He did not have metastatic lesion. In Japan,40 cases of Bowen's disease at genital region of male have been reported for last 10 years,and 15 cases（37.5％）changed to squamous cell carcinoma（Bowen's carcinoma）. Bowen's disease at genital region of male may have high risk of developing to squamous cell carcinoma.
