　症例は39歳男性で，2年間の不妊を主訴に当科外来を受診した。小さな精巣および左精索静脈瘤以外は外見的には正常男性で，経直腸的超音波断層法で前立腺および精嚢に異常を認めなかった。精液検査では精子を認めず，血清FSHは高値を示したが，血清LHおよびテストステロンは正常範囲内であった。染色体は45,X／46,X，＋idic（Y）（q11.2）であった。PCR法にてSRYは陽性であったが，DAZは陰性であった。患者がTESE-ICSIの施行を希望したために精巣生検を施行した。生検組織像では精細管の中にはセルトリー細胞しか認められなかった。

A 39-year-old man was referred to our clinic for closer examinations of infertility of 2 years duration. He was phenotypically a normal male except for small testes and left varicocele. Semen examination revealed azoospermia. Transrectal ultrasonography showed a normal prostate and seminal vesicles. The serum FSH was markedly elevated, while the serum LH and testosterone were within the normal ranges. G-banding chromosomal analysis revealed 45 XO/46 X+mar mosaisism. Q-banding chromosomal analysis and FISH using probe DYZ3 revealed that the marker chromosome was idic (Y) (q11.2).
