前部尿道外傷におけるMRI診断の有用性

USEFULNESS OF MAGNETIC RESONANCE IMAGING IN EVALUATION OF ANTERIOR URETHRAL INJURY

　前部尿道外傷5例に対して受傷早期と数か月後にMRI検査を行い，その有用性について検討した。受傷早期のMRI検査においては，受傷部の尿道海綿体および隣接した陰茎海綿体の出血や挫滅した部分が比較的明瞭に描出され，受傷早期の状況把握に有用であった。一方，数か月後のMRI検査においては，血腫は著明に縮小あるいは消失し，尿道および陰茎海綿体の挫滅部はT₁，T₂強調像ともに低信号となり，線維化しているものと思われた。尿道の走行や断裂部の瘢痕化の範囲などが明瞭に描出され，尿道再建術の術前評価に有用な情報をもたらした。

Five patients with traumatic anterior urethral injury were evaluated with magnetic resonance imaging (MRI) within 7 days after injury and several months later. During the early post traumatic period hematomas and disrup-tion of the anterior urethra as well as avulsion of the corpus cavernosa were clearly depicted. Several months later, hematomas were resolved and the crushed area of the anterior urethra and corpus cavernosa showed low intensity on T1 and T2-weighted images, suggesting completion of scar formation.
