フルニエ壊疽の4例

FOUR CASE REPORTS OF FOURNIER'S GANGRENE

　1983年から1994年までの12年間に当科受診のフルニエ壊疽の4例を経験した。症例1は49歳，陰嚢部発赤と腫脹を主訴に入院した。抗生剤を投与し，陰嚢皮膚の切開，排膿，壊死組織を切除，治療経過は順調であった。症例2,3，症例1と同様の処置により完治した。症例2はコカイン吸入歴2年であった。症例4は65歳，男性，罹患後の受診が遅延し，自己処置が適切でなかったため陰嚢は広範囲に壊疽を生じ，下腹部には皮下膿瘍を形成し，ショックなど重篤な状態に進展した。入院後，ショックなどに対する緊急治療を施行し，広範な外科的排膿，切除を施行し，肉芽の形成は良好であった。植皮術を施行し，良好な経過を得た。

We have experienced four cases of Fournier's gangrene in 12 years. All four cases were presented as idiopathic gangrene of scrotum and successfully treated by surgical operation and administration of antibiotics. In one case, 65-years-old man sufferred from high grade fever with extensive scrotum and abdominal wall skin necrosis and developed to shock state. He was rescued by extensive surgical resection and intensive antibiotics therapy.
