尿毒症を来した子宮脱の1例

A CASE OF PRORAPSE OF THE UTERUS WITH BILATERAL HYDRONEPHROSIS AND UREMIA

　症例は，72歳女性。子宮脱を認めたため，当院婦人科に入院し，子宮脱は翌日ペッサリー挿入し整復したが，CTにて両側水腎症，血中クレアチニンの上昇をみたため，当科転科入院した。初診時すでに尿毒症症状が出現しており，即座に左腎瘻留置し，腎後性腎不全に対する治療を行った。血中クレアチニンは漸減し，17日後には正常に復した。入院約1か月後，順行性にダブルJステントを留置し，退院外来経過観察とした。初回の入院から半年後ダブルJステントを抜去したがその後も腎機能の悪化はみられなかった。

A 72-years-old woman with uterine prorapse was admit-ted under the diagnosis of bilateral hydronephrosis and uremia in May 1994. CTscan showed bilateral severe hydronephrosis. Indwelling of percutaneous nephrostomy catheter into left renal pelvis relieved azotemia and the level of serum creatinine returned to normal in 17 days. After the improvement of her condition, double J stent was indwelled. In November 1994, double J stent was removed, but renal function remains normal.

Literatures about the mechanism of ureteral obstruction due to uterine prorapse were reviewed.
