膀胱内炭酸ガス注入下の軟性膀胱鏡検査

FLEXIBLE CYSTOSCOPY WITH CARBON DIOXIDE INSUFFLATION

　炭酸ガスを膀胱内へ注入しビデオモニター下に軟性膀胱鏡検査を行う手技を，男性142例女性15例で延べ208回施行した。適応は114回が膀胱腫瘍の再発チェック，74回が一般膀胱尿道観察，20回が膀胱鏡を使用する処置であった。炭酸ガス注入で軟性膀胱鏡により得られるテレビモニターの像は鮮明で十分に膀胱内景を観察できた。血尿や混濁尿の例では水灌流に比べて，炭酸ガス中での像が著しく鮮明であった。膀胱生検などの処置も，ガス中では出血による視野の障害がなく容易であった。

Using a flexible cystoscopy connected to video-monitor, we examined the bladder insufflated with carbon dioxide in 142 male and 15 female patients. The purpose of this examination included periodic check up of bladder can-cer (114 patients), inspection of the urethra and bladder (74 patients) and procedures to place a double J stent, to make biopsy, to inject a drug into the bladder wall and to resect tumor with a snare (20 patients). Flexible cystoscopy with carbon dioxide insufflation provided clearer views on the TV screen and faciliated endoscopic maneuvers particulary in patients with hematuria or in-fected urine.
