膀胱に発生した悪性中胚葉性混合腫瘍

MALIGNANT MESODERMAL MIXED TUMOR OF THE BLADDER: A CASE REPORT

　71歳，男性。頻尿を主訴に当科受診。膀胱鏡検査，CTで非乳頭状広基性腫瘍を認め，TURを施行。病理組織学的に，充実性の低分化癌と横紋筋肉腫様成分，軟骨肉腫様成分が混在してみられ，免疫組織化学的にも上皮性成分と非上皮性成分の存在が明らかで，悪性中胚葉性混合腫瘍と診断された。筋層への浸潤がみられたため，膀胱全摘除術を施行後，M-VAC療法を2コース行い，術後4年4か月経過した現在，再発や転移を認めていない。

A 71- year-old male with the chief complaint of polla-kisuria was revealed to have two nonpapillary sessile tumors on the trigonum and left lateral wall of the blad-der. Histopathological findings of these tumors by TUR were compatible with malignant mesodermal mixed tumor of the bladder, composed of carcinomatous and sar-comatous elements. The former consisted mainly of poorly differentiated carcinoma with occasional foci of transitional cell carcinoma and adenocarcinoma, while the latter consisted of rhabdomyosarcomatous and chon-drosarcomatous components.
