精巣水瘤の術後仮性再発を来した2例

TWO CASES OF POSTOPERATIVE FALSE RECURRENCE OF HYDROCELE OF THE TESTIS

　精巣水瘤など陰嚢内水瘤病変に対する術後の再発は非常に稀で，また仮性再発であり，最近2例の再発を経験したので報告する。症例1は21歳。右精索水瘤の診断で手術を行い精巣水瘤は認めなかったがWinkelmann手術を追加した。術後9日目水瘤の再発がみられ穿刺，エタノール注入を行うも効果なく，陰嚢ドレナージで治癒した。症例2は1歳。左精巣水瘤に対してWinkelmann手術後9日目に再発し，穿刺の効果なくエタノール注入で治癒した。仮性再発の予防には精巣の肉様膜内への固定が有効と考えられた。

Postoperative recurrence of intrascrotal hydrocele, such as hydrocele of the testis and spermatic cord, is very rare and considered false recurrence. Two cases of false recurrence of intrascrotal hydrocele were reported and postoparative complications were discussed.

Case 1 was a 21-year-old male, who was diagnosed right communicating and multilocular hydrocele of the spermatic cord without hydrocele of the testis. Radical hydrocelectomy and Winkelmann's operation were performed but recurrence of hydrocele was found on 9 postoperative days.
