　38年前に右腎結石の手術の既往がある57歳．女性。肝炎の精査で偶然，右腎中極背側より突出する石灰化を伴った直径4.5cmの円形腫瘤を指摘された。造影CTと血管造影で，腫瘤は全く造影されず，異物による腫瘤を強く疑った。腫瘤核出術を行い，術中にガーゼを確認し，腎を温存することができた。

We report a case of perirenal foreign mass of surgical sponge, suspected preoperatively and was confirmed by surgery. A 57 year-old woman, who underwent an operation for right renal stone 38 years before, was referred to us because of right perirenal mass incidentally found during examinations of hepatitis. On CT film the mass. 4.5 cm in diameter, protruded just dorsally from mid portion of the right kidney, with peripheral calcification. CT scan showed no enhancement and arteriogram revealed the mass as completely avascular. With these findings and her past history we highly suspected the mass of foreign body.
