膀胱腫瘍と紛らわしかった恥骨の骨軟骨腫

A CASE OF OSTEOCHONDROMA OF THE SYMPHYSIS PUBIS RESEMBLING A SUBMUCOSAL TUMOR OF THE BLADDER

　42歳，女性。頻尿を主訴に受診，顕微鏡的血尿を認めたため精査した。CTスキャンおよび超音波断層撮影で膀胱前壁を圧排する表面平滑な腫瘤陰影を認めた。膀胱鏡検査では前壁に腫瘤状の突出があったが粘膜に異常はなかった。膀胱粘膜下腫瘍を疑い下腹部横切開で腫瘍切除術を行った。腫瘍は恥骨結節後面より発生し，硬く表面平滑で膀胱壁を圧排していたが，癒着はなかった。病理診断は骨軟骨腫であり，恥骨原発のものとしてはまれと思われた。若干の文献的考察を加えた。

A 42-year-old female complaining of frequent urination was initially found to have microscopic hematu-ria. In cystoscopy, a mass like protrusion was seen from the anterior wall of the bladder covered by intactmucosa. Ultrasonography and computed tomography revealed a 12mm sized smooth mass pressing into theanterior wall of the bladder. It was clinically diagnosed as a submucosal tumor of the bladder and tumorresection was performed by lower abdominal incision. The tumor originated from the surface of thesymphysis pubis and was not adherent to the bladder wall. Pathological diagnosis was osteochondroma,rarely observed in pubic bone.
