バイオフィードバックを応用した逆行性尿道造影

RETROGRADE URETHROGRAPHY APPLYING BIOFEEDBACK

　逆行性尿道造影を施行する際，強い痛みや不快感，および尿道出血のみられることが多い．これらを軽減するため，バイオフィードバックの考えを応用し，患者にもテレビ透視が見えるようにしながら造影剤を注入した．96例の患者を対象として，計111回尿道造影を施行したが，初回撮影の場合疼痛は強いが，テレビ透視を見せながら行うと，痛みが軽減していた．また撮影後の出血も初回の場合に多いが，テレビ透視併用の場合，その程度は弱くなった．患者にテレビを見せながら行う尿道造影は，下腹部への注意を視覚に移し，何をされるかという不安感を消し，造影剤注入時のりきみや外括約筋の緊張をやわらげると思われた．

During the retrograde urethrography procedure, severe urethral pain, discomfort and bleeding are sometimes experienced by the patient. To reduce these side effects, we positioned the monitor used for guided fluoroscopy so that the image was visible not only to the physicians and technicians but also to the patient as an applied biofeedback technique. In 111 urethrographies performed in 96 patients, side effects were less severe when the patients were looking at the monitor, even in patients recieving urethrography for the first time.
