腎細胞癌の脊椎転移に対する後方除圧固定術

DISTRACTION ROD STABILIZATION IN THE TREATMENT OF METASTATIC RENAL CELL CARCINOMA INVOLVING SPINAL COLUMN

　我々は脊椎転移を初発症状とした腎細胞癌の2症例に対し，Harrington　ロッドを用いた脊椎固定法にLuqueの分節式椎弓下ワイヤー固定法（segmental spinal instrumenta—tion）を併用して後方除圧固定術を施行し，除圧による疼痛の寛解と脊椎支持性を再建しえた。脊椎固定術は，腫瘍の存在を受容した条件下での最大の活動力のある社会生活への，復帰につながる治療法になりえると考えた。骨破壊を伴う椎体転移をしばしば認める腎細胞癌や膀胱移行上皮癌では，脊椎固定術の適応のある症例は少なくないと考えた。

Spinal instrumentation with Harrington's rods for metastatic renal cell carcinoma involving the ver-tebral column was performed in two patients. Though spinal instruction could not be a radical treat-ment for metastatic tumors in the vertebral column, stabilization of spinal instabilitity could relief severe pain and minimize neurological damage accompanied with the destructive lesions. These benefits are invaluable from the view points of physiotherapy and nursing care.
