　21歳，男性。当院救急外来受診。外傷性横隔膜ヘルニア，腰椎脱臼骨折，右前腕両骨骨折，右腸骨骨折の診断で緊急手術施行中，右後腹膜腔に尿浸潤様浮腫が認められた。インジゴカルミンの静注で右後腹膜腔が青染した。腎実質，腎動静脈に損傷がなかったが，腎盂は完全に断裂していた。腎盂断裂吻合を行い経過は良好である。

A 21-year-old male had an emergency operation for a blunt abdominal trauma after he was pressed against concrete wall by a power shovel. Diagnosis of traumatic left diaphragma hernia and fructure of lumber vertebra, sacral bone, right radius and right ulna was made. Intraoperatively, edema at right retroperitoneum was recognised, and urological exploration revealed avulsion and laceration of right renal pelvis, which was repaired by pyeloplasty with endo-to-endo anastomosis. Postoperative IVP was satisfactory.
