巨大陳旧性陰嚢血瘤の1例

A CASE OF GIANT OBSOLETE HEMISCROTAL HEMATOCELE

　88歳，男性。左陰嚢内容の小児頭大の無痛性腫大および排尿障害を主訴に来院した。左陰嚢内血腫ならびに前立腺肥大症の診断にて，腫瘤摘出およびTUR-Pを施行した。腫瘤は680gで，壁は肥厚し古い凝血塊が充満しており組織学的に陳旧性の変化が認められた．20数年前に陰嚢部打撲の既往があり，当時の受傷を契機に発症した陰嚢血瘤が緩徐な経過をとるうちに，何らかの原因で増大した稀な症例と考えられる。

An 88-year-old male visited our hospital because of a huge left scrotal mass and dysuria. Com-plete excision of the hemiscrotal contents and TUR-P were performed. The extracted mass, weighted by 680 g, was filled with a huge amount of old blood coagula. Histological examination revealed extreme atrophy of the seminiferous tubules and obsolete hematocele. This is rare case in a veiw point that scrotal blunt blow over 20 years ago might have caused the huge hematocele.
