片側性副腎結核

A CASE OF UNILATERAL ADRENAL TUBERCULOSIS

　患者は67歳の女性で，CTにて偶然に発見された左副腎部腫瘤に内分泌非活性副腎腫瘍を疑い副腎摘出術を施行し，病理組織学的検討を行つたところ，副腎結核であつた。CTの所見では副腎の腫大とリング状の石灰化を認め，血管造影では動静脈とも萎縮を思わせ，副腎シンチでは左副腎への取り込みはみられなかつた。内分泌学的検査には異常なく赤沈の軽度の充進を認めるのみであつた。

A 67-year-old woman, complaining of the left back pain was seen with a mass shadow which had calcifications at the left adrenal region in the abdominal CT. Laboratory findings including hormonal data were within normal range. Adrenal scintigram demonstrated no RI uptake in the left adrenal gland and angiography also did not suggest hypervascular adrenal tumor.

Therefore, left adrenalectomy was performed as a nonfunctioning adrenal tumor and diagnosis of tuberculosis of the left adrenal gland was determined histopathologically.
