対麻痺患者に発生した巨大尿道憩室の1例

A CASE OF GIANT URETHRAL DIVERTICULUM IN A MALE PARAPLEGIC PATIENT

　患者は32歳の男性で排尿困難を主訴に来院。15年ほど前に交通事故による脊髄損傷のため入院となり尿道内操作をうけている。尿の排出は陰嚢に溜まつた尿を手で絞り出すように行つていた。尿道膀胱造影にて巨大な尿道憩室が描出され，本症例に対して憩室摘除術を施行した。憩室の容量は250mlと巨大であり諸家の報告でも類をみない。現在，外来通院中であるが経過は良好である。

The paraplegic patient, a 32-year-old man, complained of dysuria and swelling of the scrotum. Upon physical examination, a near fist-sized cystic mass was found in the scrotum. X-ray examination revealed a huge diverticulum in the anterior urethra. Capacity of the diverticulum was 250 ml. The patient underwent a radical excision of the diverticulum and had a good post-operative course. The urethral diverticulum of this patient was a result of an inappropriate urethral management in the early stage of spinal cord injury.
