慢性尿閉の膀胱内圧と臨床経過

CHRONIC URINARY RETENTION: CYSTOMETRY AND CLINICAL COURSE

　前立腺肥大症による慢性尿閉は膀胱内圧と臨床経過から2型に分類できた。術前，最大静止圧が40cmH₂O以上で低コンプライアンス膀胱の15例は，全例に水腎，11例に夜間尿失禁が合併していた。術後直ちに排尿状態の改善，尿失禁の消失，術後3カ月までに水腎，膀胱内圧が正常化した。一方，術前膀胱内圧が低い高コンプライアンス膀胱の4例は，残尿量は1,000ml以上あつたが，1例に水腎と尿失禁を伴つたのみであつた。水腎のない3例は，術直後は自排尿不能で排尿状態の改善に3カ月を要した。

We divided patients with chronic urinary retention into two groups according to intrinsic detrusor pressure and clinical course. Fifteen patients who had a low compliance bladder with a filling pressure of more than 40 cm water were associated with hydronephrosis. Eleven of the patients had nocturnal incontinence. All the patients responded well to surgery and recovered normal urination quickly. Pye-logram and cystometrogram returned to normal within 3 postoperative months.
