前立腺癌stage Cにおけるエストロジェン先行・放射線治療の試み

SEQUENTIAL ESTROGEN AND RADIATION THERAPY FOR STAGE C CARCINOMA OF THE PROSTATE: TRIAL TO IMPROVE THERAPEUTIC RESPONSE

　癌研における前立腺癌stage B，Cの治療成績は，1979年までのエストロジェン・放射線併用療法では5年実測生存率でstage B 91.7％，stage C 58.9％とstage Cの成績は悪かつた。原発巣が放射線治療後萎縮した症例に再燃・転移が少なく完治しているものが多い。1980年以降，stage Cの成績向上のために放射線治療に先立ち，予めエストロジェンで原発巣を縮小してから照射し，根治率を高めることを試みた結果，その局所所見に改善がみられ，萎縮に達する頻度が増加した。

Between 1980 and 1983, 10 patients with stage B carcinoma of the prostate and 10 patients with stage C were treated with megavoltage irradiation. All patients with stage B but one treated with radia-tion alone. Since the post-therapeutic course was good in those patients in whom the prostate had atro-phied after the treatment in our former experience, we tried estrogen therapy to reduce the size of the prostate for several months prior to irradiation for 8 patients with stage C carcinoma.
