陰茎癌鼠径リンパ節転移に対してバイパス手術を施行した1例

BYPASS GRAFT FOR FEMORAL ARTERY INVOLVEMENT BY METASTATIC CARCINOMA OF THE PENIS

はじめに

　陰茎癌の鼠径リンパ節転移に対して，特に大腿動静脈へ浸潤した症例に対してはあまり有効な治療方法がないのが現状であろう。今回われわれは，大腿動静脈へ浸潤した陰茎癌鼠径リンパ節転移に対して，人工血管を用いて外腸骨動脈と浅大腿動脈のバイパス手術を行い，その1週間後に腫瘍切除術および人工血管を用いて外腸骨静脈と大腿静脈のバイパス手術を施行した症例を経験したので，若干の文献的考察を加え報告する。

We report a case of metastatic carcinoma of the penis involving the deep inguinal lymph node and femoral vessels.

A 48-year-old man was admitted to our hospital because of an ulcerative mass in the left groin of 8 months' duration. The patient had a past history of penis carcinoma which had been treated by partial penectomy and inguinal lymph node dissection 4 years ago. Cytological examination of the present lesion revealed squamous cell carcinoma. For fear of rupture of femoral artery, an obturator bypass graft was performed. One week later, the groin lesion was resected and a femoral vein bypass was done.
