Congenital total hemihypertrophyを伴つた海綿腎の1例

A CASE OF MEDULLARY SPONGE KIDNEY ASSOCIATED WITH CONGENITAL TOTAL HEMIHYPERTROPHY

緒言

　海綿腎は集合管の拡張を特徴とし，尿路X線撮影での特異な造影所見により診断される疾患である。Palubinscasはurographic studyの0.5％（2,465例中14例）^1），Mayallは0.47％（2,600例中16例）^2）に海綿腎の症例をみいだしたとしている。したがつて，Kuiperらは海綿腎の頻度を人口2万人に1例程度と予想しており^3），それ程稀な疾患ではない。しかし，海綿腎には第1表^3）に示した疾患の合併が報告されており興味ある疾患といえる。われわれは最近congenital hemihyper-trophyを伴つた海綿腎の1例を経験したので報告する。

A 47-year-old man visited our clinic with the complaint of left colic pain, gross hematuria and recurrent history of spontaneous stone discharge.

A physical examination revealed asymmetric enlargment of the left half of the body. Mental retardation and other anomalies were not seen.

Urinalysis showed an acid urine with aboundant red blood cells. Twenty four-hour urinary calcium, phosphate and uric acid excretions were normal. Serum calcium, phosphate and uric acid were within normal limit.
