単純性腎嚢腫液中への抗生物質の移行について

DISTRIBUTION OF ANTIBIOTICS IN RENAL SOLITARY CYST

はじめに

　感染を合併した単純性腎嚢腫症例についてはこれまで欧米で17例^1〜3），本邦で6例^4〜7）の報告がある。この場合，強力な化学療法を施行することが第一であるが，それだけでは十分でなく，最終的には外科的治療が必要であることが経験的に知られている。われわれは化学療法のみで本症をcontrolできない理由として，腎嚢腫内には抗生物質が移行し難いのではないかと考え，この点を明らかにするために単純性腎嚢腫患者に抗生物質を静注した際の血中濃度，尿中濃度および腎嚢腫液中濃度を測定し若干の知見を得たので報告する。

The concentrations of Cefazolin sodium (CEZ) in renal solitary cyst fluid, blood and urine were measured in two patient at 2 hours after intravenous injection of 2 gr. of CEZ. The results were as follows: 3.1 and 3.2mcg/ml in cyst fluid, 62 and 125 mcg/ml in blood, and 2,000 and 8,400 mcg/ml in urine.

These findings strongly suggest that most antibiotics poorly reach to the cyst fluid, and that, if suppuration occurs within cyst, early surgical procedure should be necessary. These results also show that solitary cyst seemed to be almost disconnected with the nephron.
