観血的前立腺手術に対する麻酔方法の検討—持続硬膜外麻酔カテ2本法の利点について

ANESTHETIC METHOD FOR SUPRAPUBIC PROSTATECTOMY, WITH SPECIAL REFERENCE TO A CONTINOUS EPIDURAL ANESTHESIA WITH DOUBLE CATHETERIZATION

はじめに

　都立墨東病院に泌尿器科が開設（1967年6月）されて以来，9年6ヵ月間に198例の観血的前立腺手術が施行された。

　前立腺疾患の一つの特徴として，高齢者が多いため呼吸，循環機能障害などを合併している症例が稀でなく，その麻酔方法の選択は重要な問題である。それについては二，三の報告^1〜3）があるが，未だ満足すべき結論は得られていない。そこで198例の観血的前立腺手術のうち，169例の前立腺肥大症手術症例をえらび吸入麻酔，脊椎麻酔および硬膜外麻酔法について，術中出血量，術中，術後合併症などを比較検討した．

Suprapubic prostatectomy was performed on 164 cases with benign prostatic hyperplasia from 1967 to 1976. Anestheic methods applied on these operations were statistically analysed. The continous epidural anesthesia with double catheterization, one catheter for downward from the level between L4 and L 5, and the other for upward, Th 12 and L1, is recommended as the most suitable anesthetic method for suprapubic prostatectomy, especially for cases with poor risk and in those cases an uncontrol-lable postoprative pain might be expected. This method of anesthesia is very useful to do with an unfortunate re-operation.
