慢性前立腺炎患者におけるsplit ejaculatesの細菌培養

BACTERIAL CULTURE OF SPLIT EJACULATES OF PATIENTS WITH CHRONIC PROSTATITIS

緒言

　泌尿器科領域において慢性前立腺炎は非常に頻繁にみられる疾患にもかかわらず，その診断，治療および治癒判定がはなはだ困難なことは周知のごとくで，"Chronic Prostatitis Fact or Fiction"^1）によつて代表されるように本疾患の存在さえ疑われたことは歴史的にも有名である。

　診断に関しては，MearesとStamey^2）の発表したVB₁（first voided urine），VB₂（midstream spe-cimen），EPS（expressed prostatic secretions），VB³（urine after prostatic massage）によるfour glass　methodが現在最も広く用いられているが，Black-lock^3）のようにEPSが必ずしも問題となる病巣部由来とは限らないという理由からEPSの細菌培養の有効性を再考する向きもみられる。

Bacterial cultures of the split ejaculates from 17 patients who had a diagnosis of chronic prostatitis, and 12 healthy donors were performed in an attempt to determine whether or not bacterial cultures of first portion of the ejaculate (F-1), which represents prostatic secretion is more crucial than those of whole semen for the diagnosis of prostatitis.

Among 17 patients, gram-negative pathogenic bacteria was recovered only in one case.
